At the World Athletics Championship (Seville 1999) I spot a green-white-green horizontal triband with a red star in the central stripe. It was obviously Andalusian and, given the similarities with nationalist flags from other Spanish regions (Galician, Catalan nationalists, both with red stars over the overall design of the regional flag, and both used by left-wing nationalist movements), I suppose it could be a nationalist flag as well.

Yes, it is a leftist nationalist flag, used by political groups with socialist and communist ideas, and aims of independence for Andalusia.

The Andalusian flag (horizontal green-white-green) with a red star on the middle of the white stripe is that of Nación Andaluza (Andalusian Nation), a socialist and independentist party which took part in the past Andalusian and Spanish parliamentary elections.
