According to the 3th Report of Activities by then Interim Municipal President José Aurelio Fonseca Olivares, the flag seems to be adopted in 2015, the year is confirmed by the video reported by Flagvisioner. According to the Report, the flag was adopted by the "Reglamento para los usos protocolarios y costumbres de la Bandera del Municipio" (Regulation for protocol uses and lore of the Municipality Flag), however, the same report reads that such regulation along with other five need to be published. In Bandera del muncipio de Atotonilco there is a link that once linked to the said regulation in PDF format, now the site is down. So far, I just can't find the Regulation text. It seems it never was published in the Gazette. I have contacted for the last five years the Atotonilco el Alto authorities with no satisfactory answer.

Before 2015 until well entered 2021, basically, during the tenure of Cruz Carrillo Solís (2018-2021) another version was seen in official places. Such a version consisted of a rectangular field divided vertically in two equal parts, the one of the hoist was green and that of the fly was gold with the coat of arms in the center.
