The flag was adopted as the official town flag in October 1969 as part of the town's bicentennial celebrations. The flag has a blue design on a white background. The central portion is a reproduction of the Cohasset town seal. Above the seal is a scroll with the word "Cohasset." Underneath is the year 1770, the year Cohasset broke from Hingham and became its own town, as well as two anchors. The two anchors at either side of the seal symbolize the hope of the future, according to published reports from late 1969.

Approval was granted in 1969 to fly the town flag on the same pole as the American Flag, and for many years following the bicentennial celebrations the flag was flown on the Common for special occasions.
