The city emblem was adopted on Dec 1st 1954 and the flag was adopted on Dec 1st 1964 by Notice No 70. The white flag has a red city emblem in the center. The city emblem is a stylized kanji 小 O and hiragana の No. The circle symbolizes a reconciliation and a solidarity of the citizen and a future soar and a development. The flag proportion is 2:3.

Based with the information provided by Nozomi Kariyasu on 25 June 2014, I found Notice 70 of Dec 1st, 1964 (Showa 39) and it does contain a construction sheet. The ratio of 2x3 is confirmed and the emblem is centered on the white background. The ratio of the emblem is 7/10th of the width of the flag.
