Here is the flag of the commune of Ostrów Wielkopolski, in the county with the same name, Wielkopolskie vojvodship. It is presented at the communal website.
Flag description: green, a red fly-diagonal of (c) 1/4 flag-height, Trinidad-style.
Coat of Arms: per fess a) gules a horse iron or; b) checky gules and or, a bull's head noir naissant crowned or. (is there a name for it?).

Yes there is, in fact there are two established vexillological ways to describe such a flag recorded in the Dictionary of Vexillology. The one I prefer is "south-north diagonal" (with the reverse being a "north-south diagonal"), and the alternative is an "enhanced bend sinister" (with the reverse being a "bend sinister"). The latter were established by Znamierowski, but the reason that I prefer the former is that these last (to my mind) distort the heraldic terms slightly.
